Medical Forms:   

Please email a summary of medical requirements and any relevant action plans to directors@wollangarra.org.au at least 2 weeks prior to the course commencement.

Please also print 2 hardcopies of each persons medical form and ensure these are brought to Wollangarra with the teachers/staff. Please note brining hard copies of the medical forms is important as we are often in areas without reception and our printing capacity at the homestead is limited. 

Please Note: We need to receive medical forms from all attendees including teachers, staff and any parent/guardian helpers. Thanks!

Application and Permission Form:

Please ensure that permission forms are filled out for all students (under 18s) and are signed by a parent/guardian. Please email all filled and signed forms to directors@wollangarra.org.au at least 2 weeks prior to the course commencement.
